Federal law requires the Notus School District, upon request, to release the name, home address, email, and telephone number of all high school Juniors and Seniors to military recruiters unless the student or the student’s parent/guardian has directed the school not to release this information. If you do NOT want the school district to release this information to military recruiters, please fill out and submit the form below.

Once received, the student’s name will be removed from the list of student names released to military recruiters.

By signing below I provide the Notus School District with notice that it does NOT have permission to disclose any personal data about the below-named student to any and all branches of the military and/or their recruiters. This opt out form is good for the 2026-2027 school year only and does not roll over to the next year.
